Adoption Application |

Thank you for your interest in adopting a rescued malamute from the Alaskan Malamute HELP League (AMHL).
In order to be considered as an adopter you must:

be 21 years of age or older;
have identification showing your present address;
have the knowledge and consent of your landlord, or alternatively, your family;
be able and willing to spend the time and money necessary to provide training, medical treatment and proper
care for a pet.

This form is an Adoption Application -- not an agreement to adopt a dog. We will use your answers as a starting
point to work with you to determine whether or not a malamute is really the dog you are looking for. If so,
your completed application will provide the information we need to help you select a pet who will fit in well
with your family, lifestyle and expectations.
The completion of this application is the first step toward approval as an AMHL adopter. In addition, you
should:

Notify all references whom you list on this application, including veterinary clinics and private individuals
giving them your permission to speak with an AMHL representative when contacted
Expect to be contacted by an AMHL volunteer with additional questions that will help us in processing
your application.
Cooperate in scheduling a home visit. (with all members of your family present)

There is an adoption fee for all dogs adopted through the AMHL of $300.00 for healthy dogs aged seven and under or $75.00 for dogs over the age of seven.
The AMHL does not charge a fee for shelter or owner referrals. In the case of dogs who remain in shelters or with owners until they are placed,
all contractual arrangements are made directly between the shelter/owner and the adopter.

It is our practice to place each dog with the applicant whose home,
lifestyle, and family situation best fits his/her needs. While we cannot
guarantee that the dog you are initially interested in will ultimately be
placed with you, we will, once your application is approved, seriously
consider your home for the dog or dogs you indicate below.
